Prescription Refills

Please bring your medication list with you to each appointment. We prefer to refill your medications at the time of your office visit. Prescription refill is not an emergency, therefore refills will not be placed after hours.

Please leave the following information on the line:

• Date of Birth
• Your Phone Number
• Name of Medication
• Preferred Pharmacy Name
• Pharmacy Phone Number

Allow 48 hours for prescriptions to be called in or faxed to the pharmacy. Please call the pharmacy first to see if your order has been called in before calling the automated line again.

We are unable to repeatedly refill medications long term without an office visit so that we can recheck your condition.

Requesting Medical Records

Requests for medical records can be faxed to 706-860-2063. Your records are kept confidential. We will not release any of your information without first obtaining a signed authorization from you.
